Overview
The conference is designed to equip primary care clinicians with the latest evidence-based guidelines, practical tools, and clinical strategies for the prevention and management of cardiovascular disease and its risk factors. Through expert-led sessions and case-based learning, the conference aims to enhance clinicians’ confidence in delivering high-quality, patient-centered cardiovascular care. In addition, the program will highlight the breadth of expertise, multidisciplinary collaboration, and clinical resources available within the Hartford HealthCare Heart & Vascular Institute, fostering stronger connections between primary care and specialty services to improve patient outcomes across the continuum of care.

Objectives
As a result of participating in this activity, learners should be able to:

  1. Apply current evidence-based guidelines to the prevention, diagnosis, and management of common cardiovascular diseases encountered in primary care.
  2. Identify and manage key cardiovascular risk factors—including hypertension, hyperlipidemia, diabetes, and obesity—using practical, guideline-directed strategies.
  3. Interpret and incorporate emerging clinical evidence into everyday decision-making to improve patient outcomes.
  4. Utilize case-based reasoning to evaluate and manage complex cardiovascular presentations in the outpatient setting.
  5. Enhance patient-centered care approaches, including shared decision-making and individualized treatment planning for diverse patient populations.
  6. Recognize indications for specialty referral and effectively coordinate care with cardiology and other disciplines.
  7. Leverage multidisciplinary resources and services available within Hartford HealthCare to optimize continuity and quality of care.
  8. Strengthen collaboration between primary care and specialty providers to improve transitions of care and long-term cardiovascular outcomes.
  9. Increase clinical confidence in managing cardiovascular disease across the continuum—from prevention to chronic disease management.

In support of improving patient care, this activity has been planned and implemented by Hartford HealthCare and the Connecticut Chapter of the American College of Cardiology.  Hartford HealthCare is jointly accredited by the Accreditation Council for Continuing Medical Education (ACCME), the Accreditation Council for Pharmacy Education (ACPE), and the American Nurses Credentialing Center (ANCC), to provide continuing education for the healthcare team.

  • Physicians: Hartford HealthCare designates this Live Activity for a maximum of 6.00 AMA PRA Category 1 Credit™.  Physicians should claim only the credit commensurate with the extent of their participation in the activity.

    • ABIM MOC: Successful completion of this CME activity, which includes participation in the evaluation component, enables the participant to earn up to 6.00 Medical Knowledge and Patient Safety credits in the American Board of Internal Medicine's (ABIM) Maintenance of Certification (MOC) program. Participants will earn MOC points equivalent to the amount of CME credits claimed for the activity. It is the CME activity provider's responsibility to submit participant completion information to ACCME for the purpose of granting ABIM MOC credit.

  • Nurses: This activity is designated for a maximum of 6.00 ANCC contact hours.

  • Interprofessional Continuing Education: This activity was planned by and for the healthcare team, and learners will receive 6.00 Interprofessional Continuing Education (IPCE) credit for learning and change.  

  • All Other Learners: All other learners will receive a Certificate of Participation for a maximum of 6.00 hours of education. Consult your professional licensing board regarding the applicability and acceptance of credits certified by an organization accredited by Joint Accreditation for Interprofessional Continuing Education.

Cancellations received in writing no less than 5 business days before the course will receive a full refund.  No refunds will be made for cancellations received after that date.  All refunds will be made in accordance with the procedures of the HHC Accounts Payable Department, and may take up to 60 days.  Please send cancellation requests to continuinged@hhchealth.org.
