Care Planning in End-Stage Heart Failure (Session A)

This education is designed to equip clinical leaders and advanced practice providers with practical strategies to reduce avoidable readmissions in patients with end-stage heart failure. It focuses on earlier, more effective integration of palliative care and advance care planning to ensure care aligns with patient goals and preferences. Participants will review best practices in care coordination, communication, and evidence-based interventions that support smoother transitions across care settings. Ultimately, the program aims to enhance quality of care, improve patient and family experience, and drive better clinical and System-level outcomes.

1. Identify high impact drivers of Heart Failure readmissions and apply evidence based, interdisciplinary strategies to reduce preventable returns to the hospital.
2. Recognize appropriate timing and triggers for palliative care and advanced care planning in patients with advanced Heart Failure, beyond end of life scenarios.
3. Lead and support goal concordant care conversations that align treatment decisions with patient values while coordinating effectively across cardiology, inpatient, and palliative care teams.

• Physicians: Hartford HealthCare designates this Live Activity and Enduring Material for a maximum of 1.00 AMA PRA Category 1 Credit™.  Physicians should claim only the credit commensurate with the extent of their participation in the activity.

  • ABIM MOC: Successful completion of this CME activity, which includes participation in the evaluation component, enables the participant to earn up to 1.00 Medical Knowledge and Patient Safety credits in the American Board of Internal Medicine's (ABIM) Maintenance of Certification (MOC) program. Participants will earn MOC points equivalent to the amount of CME credits claimed for the activity. It is the CME activity provider's responsibility to submit participant completion information to ACCME for the purpose of granting ABIM MOC credit.

• Nurses: This activity is designated for a maximum of 1.00 ANCC contact hours.

• Interprofessional Continuing Education: This activity was planned by and for the healthcare team, and learners will receive 1.00 Interprofessional Continuing Education (IPCE) credit for learning and change.

Cancellation Policy

Cancellations received in writing no less than 5 business days before the course will receive a full refund.  No refunds will be made for cancellations received after that date.  All refunds will be made in accordance with the procedures of the HHC Accounts Payable Department, and may take up to 60 days.  Please send cancellation requests to continuinged@hhchealth.org.
