This program will address the critical inter-professional skill of communication in the clinical environment. Topics addressed include evidence -based skills for relationship centered care in the context of high quality primary palliative care in all HHC Clinical environments. Topics include caring for specialty populations, best practices for holistic care of patients living with serious illness, symptom management, advance care planning and care for the caregiver.


Objectives
As a result of participating in this activity, learners should be able to:

  1. Articulate evidence based communication strategies to increase clinician confidence in creating relationship centered interactions and clinical experiences
  2. Examine a best practice model for primary & specialty palliative care in the congestive heart failure population and communication techniques with those facing serious illness.
  3. Explore evidence based holistic, patient and family centered strategies in care of patients with dementia.
  4. Identify evidence based strategies for best supportive care (primary & specialty) across the care continuum for those living with a cancer diagnosis
  5. Define quality of life focused, evidence based interventions for the care of patients experiencing lymphedema.
  6. Explore the resources and shared governance of the HHC Palliative Care Council and the opportunities for individual colleague engagement.
  7. Define the elements of high quality palliative care and articulate why it meets the needs of our healthcare population

In support of improving patient care, Hartford HealthCare is jointly accredited by the Accreditation Council for Continuing Medical Education (ACCME), the Accreditation Council for Pharmacy Education (ACPE), and the American Nurses Credentialing Center (ANCC) to provide continuing education for the healthcare team.

  • Physicians: Hartford HealthCare designates this Live Activity for a maximum of 6.00 AMA PRA Category 1 Credit™.  Physicians should claim only the credit commensurate with the extent of their participation in the activity.
    • ABIM MOC: Successful completion of this CME activity, which includes participation in the evaluation component, enables the participant to earn up to 6.00 Medical Knowledge credits in the American Board of Internal Medicine's (ABIM) Maintenance of Certification (MOC) program. Participants will earn MOC points equivalent to the amount of CME credits claimed for the activity. It is the CME activity provider's responsibility to submit participant completion information to ACCME for the purpose of granting ABIM MOC credit.
    •  ABS Continuous Certification: Successful completion of this CME activity, which includes participation in the evaluation component, enables the learner to earn credit toward the CME and Self-Assessment requirements of the American Board of Surgery’s Continuous Certification program. It is the CME activity provider's responsibility to submit learner completion information to ACCME for the purpose of granting ABS credit.
  • Psychologists: Continuing Education (CE) credits for psychologists are provided through the co-sponsorship of the American Psychological Association (APA) Office of Continuing Education in Psychology (CEP). The APA CEP Office maintains responsibility for the content of the programs. This activity is designated for a maximum of 6.00 APA CEP credits. 
  • Nurses: This activity is designated for a maximum of 6.00 ANCC contact hours.
  • Social Workers: As a Jointly Accredited Organization, Hartford HealthCare is approved to offer social work continuing education by the Association of Social Work Boards (ASWB) Approved Continuing Education (ACE) program.  Organizations, not individual courses, are approved under this program. State and provincial regulatory boards have the final authority to determine whether an individual course may be accepted for continuing education credit.  Hartford HealthCare maintains responsibility for this course.  Social workers completing this course receive a maximum of 6.00 general continuing education credits.
  • All Other Learners: All other learners will receive a Certificate of Participation for a maximum of 6.00 hours of education. Consult your professional licensing board regarding the applicability and acceptance of certificates of participation for educational activities certified for AMA PRA Category 1 Credit™ from organizations accredited by Joint Accreditation for Interprofessional Continuing Education.

Cancellations received in writing no less than 5 business days before the course will receive a full refund.  No refunds will be made for cancellations received after that date.  All refunds will be made in accordance with the procedures of the HHC Accounts Payable Department, and may take up to 60 days.  Please send cancellation requests to continuinged@hhchealth.org.
