Overview
Prioritizing equity in everything that we do creates the context for a community where individuals are equipped to live their healthiest lives. It ensures people are granted equal opportunity to access care despite social, economic, and physical factors. Disparities in health care adversely affect groups facing other inequities and often limit the ability to improve quality of life, immensely contributing to poorer outcomes and unnecessary costs.


Objectives
As a result of participating in this activity, learners should be able to:

  1. Define equity as it relates to health disparities
  2. Understand public policy and advocacy addressing health disparities
  3. Articulate how inequities in healthcare affect outcomes and safety
  4. Learn how to scale a community-focused approach to provide equitable care
  5. Hear about a few research-informed interventions designed to address disparities and improve outcomes

Accreditation
In support of improving patient care, Hartford HealthCare is jointly accredited by the Accreditation Council for Continuing Medical Education (ACCME), the Accreditation Council for Pharmacy Education (ACPE), and the American Nurses Credentialing Center (ANCC) to provide continuing education for the healthcare team.
  • Physicians: Hartford HealthCare designates this activity for a maximum of 5.00 AMA PRA Category 1 Credit(s)TM.  Physicians should claim only the credit commensurate with the extent of their participation in the activity.  
  • Nurses: This activity is designated for a maximum of 5.00 ANCC contact hours.
  • Other Learners: All other learners will receive a Certificate of Participation.  Consult your professional licensing board regarding the applicability and acceptance of certificates of participation for educational activities certified for AMA PRA Category 1 Credit™ from organizations accredited by Joint Accreditation for Interprofessional Continuing Education.

Cancellation Policy

Cancellations received in writing no less than 5 business days before the course will receive a full refund.  No refunds will be made for cancellations received after that date.  All refunds will be made in accordance with the procedures of the HHC Accounts Payable Department, and may take up to 60 days.  Please send cancellation requests to [email protected]hhchealth.org.
