Overview
Solid organ transplantation has become the standard of care for individuals with end-stage kidney, liver, or heart disease. There are more than 107,000 people in the United States on the United Network for Organ Sharing wait list. Advances through research have significantly impacted the success rates of transplantation, resulting in increased life years and improved quality of life for transplant recipients. Care of patients pre and post transplant is conducted in collaboration with community providers. Participants who attend this symposium will be able to apply updated therapies to the care of individuals with end-stage organ disease.


Objectives
As a result of participating in this activity, learners should be able to:

  1. Identify lifestyle modifications to optimize health in the older adult population prior to transplant
  2. Define barriers to transplant based on frailty.
  3. Identify the social determinants of Health and the definition of healthcare disparities.
  4. Explain how GFR equations differentially influence access to renal care and kidney transplantation across racial groups.
  5. Detect likelihood of bias in transplant evaluation with regards to substance use vs misuse.
  6. Discuss patient's options when transplant is no longer a viable option.
  7. Review the process of standard selection in transplant.
  8. Identify the physiologic changes associated with normal aging in relation to immunosuppression and anti-infective absorption, distribution, metabolism, and excretion.
  9. Apply strategies to enhance the safety, effectiveness and adherence of transplant medications.
  10. Identify the role of palliative care in the transplant population.
  11. Examine where subconscious or unconscious bias may exist.
  12. Define the ethical principles of efficiency and equity as related to organ procurement and allocation.

Accreditation
In support of improving patient care, Hartford HealthCare is jointly accredited by the Accreditation Council for Continuing Medical Education (ACCME), the Accreditation Council for Pharmacy Education (ACPE), and the American Nurses Credentialing Center (ANCC) to provide continuing education for the healthcare team.
  • Physicians: Hartford HealthCare designates this Live Activity activity for a maximum of 6.50 AMA PRA Category 1 Credit™.  Physicians should claim only the credit commensurate with the extent of their participation in the activity.  
  • Nurses: This activity is designated for a maximum of 6.50 ANCC contact hours.

  • Social Workers: As a Jointly Accredited Organization, Hartford HealthCare is approved to offer social work continuing education by the Association of Social Work Boards (ASWB) Approved Continuing Education (ACE) program. Organizations, not individual courses, are approved under this program. Regulatory boards are the final authority on courses accepted for continuing education credit. Social workers completing this course receive 6.50 general continuing education credits.
  • Dieticians: In Process
  • Certified Clinical Transplant Coordinator: The ABTC Board of Governors has granted a total of 6.50 Category I Continuing Education Points for Transplant Certification (CEPTC) to the 16th Annual Interdisciplinary Transplant Symposium.
  • Other Learners: All other learners will receive a Certificate of Participation for 6.50 hours of education.  Consult your professional licensing board regarding the applicability and acceptance of certificates of participation for educational activities certified for AMA PRA Category 1 Credit™ from organizations accredited by Joint Accreditation for Interprofessional Continuing Education.

Cancellation Policy

Cancellations received in writing no less than 5 business days before the course will receive a full refund.  No refunds will be made for cancellations received after that date.  All refunds will be made in accordance with the procedures of the HHC Accounts Payable Department, and may take up to 60 days.  Please send cancellation requests to [email protected]hhchealth.org.
