Death Over Dinner ... Let's Talk

Death Over Dinner is a two-hour educational activity designed to enhance provider knowledge, confidence, and communication skill in facilitating conversations related to end-of-life planning. Based on the national Death Over Dinner model, this discussion-based activity provides a structured environment for examining topics of mortality, legacy, advance care planning, and advance directives. Through guided reflection and facilitated dialogue, participants will explore personal and professional barriers to these discussions and strengthen their ability to engage patients and families in compassionate, patient-centered goals-of-care conversations.  Death Over Dinner TedTalk

2. Discuss barriers and facilitators that influence provider comfort and effectiveness in end-of-life conversations

Physicians: Hartford HealthCare designates this live actrivity for a maximum of 2.00 AMA PRA Category 1 Credit™.  Physicians should claim only the credit commensurate with the extent of their participation in the activity.

ABIM MOC: Successful completion of this CME activity, which includes participation in the evaluation component, enables the participant to earn up to 2.00 Medical Knowledge credits in the American Board of Internal Medicine's (ABIM) Maintenance of Certification (MOC) program. Participants will earn MOC points equivalent to the amount of CME credits claimed for the activity. It is the CME activity provider's responsibility to submit participant completion information to ACCME for the purpose of granting ABIM MOC credit,

ABP MOC: Successful completion of this CME activity, which includes participation in the evaluation component, enables the learner to earn up to 2.00 MOC points in the American Board of Pediatrics' (ABP) Maintenance of Certification (MOC) program. It is the CME activity provider's responsibility to submit learner completion information to ACCME for the purpose of granting ABP MOC credit.

ABS Continuous Certification: Successful completion of this CME activity which includes participation in the evaluation component enables the learner to earn 2.00 credit toward the CME and Self-Assessment requirements of the American Board of Surgery’s Continuous Certification program. It is the CME activity provider's responsibility to submit learner completion information to ACCME for the purpose of granting ABS credit.

Pharmacists & Pharmacy Techs: This activity is approved for a maximum of 2.00 CPE contact hours. ACPE UAN: JA4008261-0000-26-067-L99-P and JA4008261-0000-26-067-L99-T.  Awarding of credit is based on completing an activity evaluation form.

Nurses: This activity is designated for a maximum of 2.00 ANCC contact hours.

Psychologists: Continuing Education (CE) credits for psychologists are provided through the co-sponsorship of the American Psychological Association (APA) Office of Continuing Education in Psychology (CEP). The APA CEP Office maintains responsibility for the content of the programs. This activity is designated for a maximum of 2.00 APA CEP credits. 

Social Workers: As a Jointly Accredited Organization, Hartford HealthCare is approved to offer social work continuing education by the Association of Social Work Boards (ASWB) Approved Continuing Education (ACE) program.  Organizations, not individual courses, are approved under this program. State and provincial regulatory boards have the final authority to determine whether an individual course may be accepted for continuing education credit.  Hartford HealthCare maintains responsibility for this course.  Social workers completing this course receive a maximum of 2.00 clinical continuing education credits.

Interprofessional Continuing Education: This activity was planned by and for the healthcare team, and learners will receive 2.00 Interprofessional Continuing Education (IPCE) credit for learning and change.

All Other Learners: All other learners will receive a Certificate of Participation for a maximum of 2.00 hours of education. Consult your professional licensing board regarding the applicability and acceptance of credits certified by an organization accredited by Joint Accreditation for Interprofessional Continuing Education.

Cancellation and Refund Policy

• HHC or the sponsoring organization reserves the right to cancel, postpone, or modify this activity
• If the activity is cancelled, registration fees will be fully refunded
• Neither HHC, HHC CE, not the Sponsoring Organization is responsible for travel, lodging, or other incurred expenses due to a cancelled activity
• All cancellation requests must be submitted in writing to HHC CE at [email protected]
• Approved refunds will be processed to the original form of payment within 30 days 

 Live (In Person or Virtual) Activity Cancellations

• Cancellations received at least 5 business days prior to the activity start date will receive a full refund
• Cancellations received less than 5 business days prior to the activity start date, or failure to attend the activity absent an illness or family emergency (no-show), are non-refundable 

 On-Demand / Enduring Activities

• Registration fees are non-refundable once access is granted
