Reimagining Value: A Look Into Clinical Care Redesign

Focusing on best practice, leveraging analytics to increase patient safety, improve outcomes and decrease variation throughout the system.

1. Review how to advance clinical care in a challenging financial environment
2. Describe how changes in practice can lead to quality improvements and/or cost savings
3. Describe the improtance of partnerships between supply chain and clinical service lines
4. Apply data to make clinical care updates consistent with best practice while decreasing variation and cost of care, increasing patient safety, and improving outcomes
5. Discuss current projects for clinical care redesign, including the source of the project as well as the data and how they were successfully executed
6. Review new processes implemented that have impacted patient safety, quality outcomes, and cost of care

In support of improving patient care, Hartford HealthCare is jointly accredited by the Accreditation Council for Continuing Medical Education (ACCME), the Accreditation Council for Pharmacy Education (ACPE), and the American Nurses Credentialing Center (ANCC) to provide continuing education for the healthcare team.

• Physicians: Hartford HealthCare designates this Live Activity for a maximum of 2.50 AMA PRA Category 1 Credit™.  Physicians should claim only the credit commensurate with the extent of their participation in the activity.
• Nurses: This activity is designated for a maximum of 2.50 ANCC contact hours.

Cancellation Policy

Cancellations received in writing no less than 5 business days before the course will receive a full refund.  No refunds will be made for cancellations received after that date.  All refunds will be made in accordance with the procedures of the HHC Accounts Payable Department, and may take up to 60 days.  Please send cancellation requests to continuinged@hhchealth.org.

Hartford HealthCare reserves the right to cancel or postpone this program if necessary; in the event of cancellation, course fees will be fully refunded. We are not responsible for other costs incurred such as non-refundable airline tickets or hotel penalties.
