Topics addressed include evidence based skills for successful patient and family discussions including; initial determination of patient's goals and preferences, shared decision making and motivational interviewing. Special populations will also be discussed as well as the importance of delivering early high quality primary palliative care.

Objectives
As a result of participating in this activity, learners should be able to:

  1. Articulate evidence based communication strategies to increase clinician confidence in creating relationship centered interactions
  2. Explore the language of prognostication and its impact on the patient family experience
  3. Define the foundational elements of primary palliative care and its impact on quality and safety and patient experience
  4. Identify evidecne based strategies to communicate effectively with unique patient populations

Accreditation

In support of improving patient care, Hartford HealthCare is jointly accredited by the Accreditation Council for Continuing Medical Education (ACCME), the Accreditation Council for Pharmacy Education (ACPE), and the American Nurses Credentialing Center (ANCC) to provide continuing education for the healthcare team.

  • Physicians: Hartford HealthCare designates this Live Activity for a maximum of 5.75 AMA PRA Category 1 Credit™. Physicians should claim only the credit commensurate with the extent of their participation in the activity.
    • ABIM MOC: Successful completion of this CME activity, which includes participation in the evaluation component, enables the participant to earn up to 5.75 Medical Knowledge credits in the American Board of Internal Medicine's (ABIM) Maintenance of Certification (MOC) program.  Participants will earn MOC points equivalent to the amount of CME credits claimed for the activity. It is the CME activity provider's responsibility to submit participant completion information to ACCME for the purpose of granting ABIM MOC credit.
    • ABP MOC: Successful completion of this CME activity, which includes participation in the evaluation component, enables the learner to earn up to 5.75 MOC points in the American Board of Pediatrics' (ABP) Maintenance of Certification (MOC) program.  It is the CME activity provider's responsibility to submit learner completion information to ACCME for the purpose of granting ABP MOC credit.
    • ABS Continuous Certification and Self Assessment : Successful completion of this CME activity, which includes participation in the evaluation component, enables the learner to earn credit toward the CME and Self-Assessment requirements of the American Board of Surgery’s Continuous Certification program. It is the CME activity provider's responsibility to submit learner completion information to ACCME for the purpose of granting ABS credit.
  • Nurses: This activity is designated for a maximum of 5.75 ANCC contact hours.
  • Social Workers: As a Jointly Accredited Organization, Hartford HealthCare is approved to offer social work continuing education by the Association of Social Work Boards (ASWB) Approved Continuing Education (ACE) program. Organizations, not individual courses, are approved under this program. State and provincial regulatory boards have the final authority to determine whether an individual course may be accepted for continuing education credit. Hartford HealthCare maintains responsibility for this course. Social workers completing this course receive 5.75 general continuing education credits.
  • All Other Learners: All other learners will receive a Certificate of Participation for 5.75 hours of education. Consult your professional licensing board regarding the applicability and acceptance of certificates of participation for educational activities certified for AMA PRA Category 1 Credit™ from organizations accredited by Joint Accreditation for Interprofessional Continuing Education.

Cancellation Policy

Cancellations received in writing no less than 5 business days before the course will receive a full refund.  No refunds will be made for cancellations received after that date.  All refunds will be made in accordance with the procedures of the HHC Accounts Payable Department, and may take up to 60 days.  Please send cancellation requests to continuinged@hhchealth.org.
