The Hartford HealthCare Heart & Vascular Institute Lifestyle Medicine Conference will bring awareness to the growing field of lifestyle medicine and its application to everyday practice that aims to address and reverse patients’ chronic diseases through its six pillars, which include plant-based nutrition, physical activity, stress management, avoidance of risky substances, restorative sleep, and social connections.


Objectives
As a result of participating in this activity, learners should be able to:

  1. Review the societal and economic burden of lifestyle-related chronic diseases and the need for interventions
  2. Define lifestyle medicine and illustrate six key pillars used to treat lifestyle-related chronic conditions
  3. Utilize lifestyle medicine principles/pillars to guide patients effectively in the prevention, treatment and reversal of disease
  4. Identify and apply lifestyle medicine evidence-based recommendations to clinical practice
  5. Enhance efficacy in assessment of patients’ lifestyle and supporting lifestyle change
  6. Explore components of a lifestyle medicine practice
  7. Discuss scientific evidence demonstrating stabilization and reversal of cardiovascular disease and diabetes
  8. Describe the HHC Lifestyle Medicine Program

In support of improving patient care, Hartford HealthCare is jointly accredited by the Accreditation Council for Continuing Medical Education (ACCME), the Accreditation Council for Pharmacy Education (ACPE), and the American Nurses Credentialing Center (ANCC) to provide continuing education for the healthcare team.

  • Physicians: Hartford HealthCare designates this Live Activity for a maximum of 5.75 AMA PRA Category 1 Credit™.  Physicians should claim only the credit commensurate with the extent of their participation in the activity.

    • ABIM MOC: Successful completion of this CME activity, which includes participation in the evaluation component, enables the participant to earn up to 5.75 Medical Knowledge credits in the American Board of Internal Medicine's (ABIM) Maintenance of Certification (MOC) program. Participants will earn MOC points equivalent to the amount of CME credits claimed for the activity.  It is the CME activity provider's responsibility to submit participant completion information to ACCME for the purpose of granting ABIM MOC credit.

  • Psychologists: Continuing Education (CE) credits for psychologists are provided through the co-sponsorship of the American Psychological Association (APA) Office of Continuing Education in Psychology (CEP).  The APA CEP Office maintains responsibility for the content of the programs.

  • Nurses: This activity is designated for a maximum of 5.75 ANCC contact hours.

  • Dietitians and Dietetic Technicians, Registered: Completion of this RD/DTR profession-specific or IPCE activity awards CPEUs (One IPCE credit = One CPEU).  If the activity is dietetics-related but not targeted to RDs or DTRs, CPEUs may be claimed which are commensurate with participation in contact hours (One 60 minute hour = 1 CPEU).  RD's and DTRs are to select activity type 102 in their Activity Log.  Performance Indicator selection is at the learner's discretion.

  • Social Workers: As a Jointly Accredited Organization, Hartford HealthCare is approved to offer social work continuing education by the Association of Social Work Boards (ASWB) Approved Continuing Education (ACE) program. Organizations, not individual courses, are approved under this program. Regulatory boards are the final authority on courses accepted for continuing education credit. State and provincial regulatory boards have the final authority to determine whether an individual course may be accepted for continuing education credit. Hartford HealthCare maintains responsibility for this course. Social workers completing this course receive 5.75 general continuing education credits.

  • All Other Learners: All other learners will receive a Certificate of Participation for 5.75 hours of education.  Consult your professional licensing board regarding the applicability and acceptance of certificates of participation for educational activities certified for AMA PRA Category 1 Credit™ from organizations accredited by Joint Accreditation for Interprofessional Continuing Education.



Cancellation Policy

Cancellations received in writing no less than 5 business days before the course will receive a full refund.  No refunds will be made for cancellations received after that date.  All refunds will be made in accordance with the procedures of the HHC Accounts Payable Department, and may take up to 60 days.  Please send cancellation requests to continuinged@hhchealth.org.
