LOCATION: The conference is being held via Teams, or in person at Heublein Hall in the Education and Resource Center, Hartford Hospital, 560 Hudson St.  Instructions from parking to Heublein Hall or the Teams link will be provided via email the day prior to the conference.

Diabetes affects over 38 million children and adults in the US. People with diabetes are at high risk of serious health complications such as stroke, blindness, kidney disease, heart disease, nerve damage and amputations. The symposium will feature the most up-to-date information on diabetes management and treatment advancements for providers and healthcare professionals in the primary care space.

1. Apply current recommendations for diabetes management in the primary care space.
2. Discuss current advancements in diabetes management in primary care.
3. Educate patients in prevention of diabetes-related complications

In support of improving patient care, Hartford HealthCare is jointly accredited by the Accreditation Council for Continuing Medical Education (ACCME), the Accreditation Council for Pharmacy Education (ACPE), and the American Nurses Credentialing Center (ANCC) to provide continuing education for the healthcare team.

• Physicians: Hartford HealthCare designates this Live Activity for a maximum of 6.00 AMA PRA Category 1 Credit™.  Physicians should claim only the credit commensurate with the extent of their participation in the activity.

  • ABIM MOC: Successful completion of this CME activity, which includes participation in the evaluation component, enables the participant to earn up to 6.00 Medical Knowledge credits in the American Board of Internal Medicine's (ABIM) Maintenance of Certification (MOC) program. Participants will earn MOC points equivalent to the amount of CME credits claimed for the activity. It is the CME activity provider's responsibility to submit participant completion information to ACCME for the purpose of granting ABIM MOC credit.

• Nurses: This activity is designated for a maximum of 6.00 ANCC contact hours.

• Pharmacists & Pharmacy Techs: This activity is approved for a maximum of 6.00 CPE contact hours. ACPE UAN: TBD and TBD.  Awarding of credit is based on completing an activity evaluation form.

• Dietitians and Dietetic Technicians, Registered: Completion of this RD/DTR profession-specific or IPCE activity awards CPEUs (One IPCE credit = One CPEU).  If the activity is dietetics-related but not targeted to RDs or DTRs, CPEUs may be claimed which are commensurate with participation in contact hours (One 60 minute hour = 1 CPEU).  RD's and DTRs are to select activity type 102 in their Activity Log.  Performance Indicator selection is at the learner's discretion. This activity is designated for a maximum of 6.00 CDR CPEUs. 

• Social Workers: As a Jointly Accredited Organization, Hartford HealthCare is approved to offer social work continuing education by the Association of Social Work Boards (ASWB) Approved Continuing Education (ACE) program. Organizations, not individual courses, are approved under this program. Regulatory boards are the final authority on courses accepted for continuing education credit. State and provincial regulatory boards have the final authority to determine whether an individual course may be accepted for continuing education credit. Hartford HealthCare maintains responsibility for this course. Social workers completing this course receive 6.00 general continuing education credits.

• All Other Learners: All other learners will receive a Certificate of Participation for a maximum of 6.00 hours of education. Consult your professional licensing board regarding the applicability and acceptance of certificates of participation for educational activities certified for AMA PRA Category 1 Credit™ from organizations accredited by Joint Accreditation for Interprofessional Continuing Education.

Cancellation and Refund Policy

• HHC or the sponsoring organization reserves the right to cancel, postpone, or modify this activity

• If the activity is cancelled, registration fees will be fully refunded

• Neither HHC, HHC CE, not the Sponsoring Organization is responsible for travel, lodging, or other incurred expenses due to a cancelled activity

• All cancellation requests must be submitted in writing to HHC CE at [email protected]

• Approved refunds will be processed to the original form of payment within 30 days 

Participant Cancellations

• Cancellations received at least 5 business days prior to the activity start date will receive a full refund

• Cancellations received less than 5 business days prior to the activity start date, or failure to attend the activity absent an illness or family emergency (no-show), are non-refundable

• Registration fees are non-refundable once access is granted
