Dr. Sean Jeffery is Director of Pharmacy Services at Integrated Care Partners, a physician-led, clinically integrated network responsible for Hartford Healthcares value-based practice transformation. Dr. Jeffery is responsible for pharmacy network development and strategy, population health management, and support of an integrated care-management team.

 

Dr. Jeffery is also a Professor of Pharmacy Practice at the University of Connecticut School of Pharmacy.  His area of expertise is senior care pharmacy. Prior to joining Integrated Care Partners, Dr. Jeffery served as a consultant pharmacist for the VA Connecticut Geriatrics Consult service from 1998 - 2015. During this time Dr. Jeffery also established a Post Graduate Year-2 Geriatrics Pharmacy Residency Program and served as Director from 2002 - 2015.

 

Dr. Jeffery has extensive association leadership experience including, currently serving as a Board of Trustee for the American Pharmacist Association (2019-22). Dr. Jeffery also served as President and Chairman of the Board of Directors for the American Society of Consultant Pharmacists (2012 – 2014), Chairman of the 2007 – 2008 American Association of Colleges of Pharmacy Geriatrics and 2013 – 2017 American Geriatrics Society Special Interest Groups.  He was also a member of the 2014 – 2016 Medication Therapy Management Technical Expert Panel (TEP) for the Center for Medicare and Medicaid Services. He has co-founded two coalitions for Senior Medication Safety in Connecticut to raise awareness among seniors about medication related problems.  The coalitions support community efforts which, target medication education/management to low-income, immigrant communities.  He presently co-chairs the State of Connecticut Medication Reconciliation and Polypharmacy Committee under the Office of Health Strategy.

 

He is a graduate of the University of Connecticut (BS R.Ph.) and Ohio State (PharmD) Universities and completed a post-graduate residency in geriatric pharmacy at the Duke Center for the Study of Aging and Durham VA Medical Center under the mentorship of Drs. Joseph Hanlon and Christine Ruby. Dr. Jeffery is a fellow of the American Society of Consultant Pharmacists and the American Geriatrics Society.

Sean Jeffery is Director of Clinical Pharmacy Services at Integrated Care Partners, an organization responsible for Hartford Healthcare Group’s value-based practice transformation.  His career has focused on implementing innovative patient care services for seniors that promote the value of pharmacists in care coordination.  Currently, he is responsible for pharmacy network development and strategy, population health management, and support of an integrated care-management team. He is also a Professor of Pharmacy Practice at the University of Connecticut School of Pharmacy. He was previously a consultant pharmacist at the VA Connecticut Healthcare System where he served as a PGY2 Geriatrics Residency Program Director.

 

Jeffery was elected to the American Pharmacist Association Board of Trustees in 2019.  He served as President and Chairman of the Board of Directors for the American Society of Consultant Pharmacists from 2012 – 2014. He is Fellow of the American Geriatrics Society (AGS) and American Society of Consultant Pharmacists.  He earned his bachelor of science from the University of Connecticut and PharmD from the Ohio State University.

Brief Introduction

 

Sean is a pharmacist who enjoys caring for seniors.  His training in geriatrics began at the Duke Center for the Study of Aging and Durham VA under Dr. Joseph Hanlon.  He then joined the University of Connecticut School of Pharmacy faculty and established a geriatric pharmacy consult service at VA Connecticut.  Over 16 years, his work at VA Connecticut included caring for home-based primary care, short-term rehabilitation, hospice, geropsychiatry and geriatric consult service patients.  He is currently a Trustee for the American Pharmacist Association and a past president of the American Society of Consultant Pharmacists. In 2015, Sean joined Hartford Healthcare’s Integrated Care Partners, as their Director of Clinical Pharmacy Services, where he applies his background in senior care across an integrated health system.  He is responsible for pharmacy network development and strategy, population health management, and support of an integrated care-management team.

Currently, he oversees pharmacy network development and strategy, population health management, and provides clinical support to the care-management team. He is also a Professor of Pharmacy Practice bat the University of Connecticut School of Pharmacy. Previously, Dr. Jeffery was a consultant pharmacist at the VA Connecticut Healthcare System where he served as a PGY2 Geriatrics Residency Program Director. Dr. Jeffery has served on the APhA Policy Committee (16), chaired the Polypharmacy Special Interest Group (SIG) for the American Geriatrics Society, chaired the American Association of Colleges of Pharmacy Geriatrics SIG, and served as President and Chairman of the Board of Directors for the American Society of Consultant Pharmacists. He was recognized as a Fellow of the American Geriatrics Society (2016), an Ohio State University Distinguished Alumni (2013). Dr. Jeffery earned his bachelor of science from the University of Connecticut and PharmD from the Ohio State University.
